What does a new patient need to bring on their first visit?

• Photo ID
• List of current medications
• Insurance cards
• New patient forms
• Any recent diagnostic testing related to current injury (x-ray, MRI, CT)

Can I still see Dr. Cohn if my insurance is out of network?

Yes, but most of the time, of the time, it will be a greater out of pocket cost to the patient.

Does Dr. Cohn take workman’s compensation?


What if I don’t have medical insurance?

Dr. Cohn sees patients without medical insurance. Full payment is due at the time of service. The cost of a new patient office visit is about $250. The cost varies due to the of treatment needed. There may be additional costs for casting, braces, injections, or X-rays.

How do I get a prescription refilled?

Call your pharmacy and request a refill on your prescription. The pharmacy will fax the information to the office to refill the prescription. It takes 24-48 hours for the office to process a refill request, so please plan accordingly. Prescription refills are not done after hours, on weekends or on Holidays.

Dr. Cohn operates at Hill Country Memorial Hospital in Fredericksburg 

How do I schedule surgery?

You may call Dr. Cohn’s office and his nurse will assist you. The nurse coordinates with the doctor’s offices, the hospital, the surgeon and your insurance carrier to pre-certify and schedule your surgery. Your surgery may need a pre-surgery physical and blood tests.

How do I know my surgery date and time?

Dr. Cohn’s nurse will send you written instructions in the mail. The hospital or surgery center will also contact you prior to surgery. Final surgery time is given by the hospital.

What if I have questions about surgery?

You may call our office during office hours to speak to a nurse.

What if I have a medical concern related to my injury or surgery?

During office hours: Call the office at 830-266-5576 to speak to a nurse.
After hours/Weekends/Holidays: Call Hill Country Memorial Hospital at 830-997-4353.

How can I get copies of my medical records?

You can send a letter to our office requesting your medical records. This letter must include the following: What type of records you are requesting (all, specific procedure, x-ray, etc.), your legal name, date of birth, social security number, and where you want these records sent.

You can also come by the office and fill out a form requesting medical records.

Please allow one week for medical records.
